Getting Started with me

Thank you for embarking on this journey with me.  In preparation for our first call, please gather this information together to help us be as productive as possible.  There 
  1. Please fill out the Parent Survey to tell me a little bit about your student.
  2. Please have your student fill out the Student Survey to help me see how they see the world.
  3. If there are potential issues with medical, psychiatric, behavioral issues or the like, please fill out my functional survey.
  4. Please gather the following documents and forward them to me in an email:
    1. School transcripts for all high school and college work (or grades from the last three years)
    2. Any test scores that you have (standardized tests, including MAP, Regents, ACT, SAT, Achievements, APs, IBs, or any other test that your student has taken in the last four years)
    3. Any memberships that your student may hold (sports teams, chess teams, musical ensembles, social clubs, worship institutions, volunteer groups, and the like).
    4. Any honors, awards, certificates that your student may hold.
    5. Any information on student's employment history that might exist.
    6. A student resume, if the student has created one.
    7. Any behavioral, psychiatric, or medical reports that may have been completed.
    8. Any letters, disciplinary or legal proceedings that may have been part of your student's history.
    9. Any other information that is helpful in getting a complete picture of the student's life and academic experiences.
  5. Please gather the following login information in one place for your reference
    1. Logins to school academic and administrative systems
    2. Logins to testing systems 
    3. Logins to college applications and financial aid systems
    4. Logins to health systems (doctors offices, hospitals, etc.)
    5. Logins to health insurance systems that may cover your student
